Here are the measurements and cuts.
Start with a 4 3/4" x 5" piece of Sweet Talk Designer Paper.
Score on the 5" side at 1", 2", 3" and 4"
Turn and score on the 4 3/4" side at 1" and 3 3/4"
Fold on score lines and cut as shown in video or photo below!
